FAFSA Verification Process
Students who have been selected for a FAFSA process called verification must verify information reported on their application. In order to do so, students must complete requested tasks at https://fsw.studentforms.com. Please monitor your student email account for updates and any additional tasks to complete the verification process.
Special Cases
For any special cases that may warrant a financial aid administrator to exercise discretion in a number of areas related to your FAFSA, you can visit https://fsw.studentforms.com and request the relevant appeal.
- Dependency override
- Change in income
- Homelessness
- Unable to provide parent information

Documents may be submitted via postal mail, fax, or in person to Student Financial Services.
